Welcome to the LVPA Forms Page. The following forms are needed for changes in guardianship and residency, for student community service, admissions and to release transcripts to other entities.
1. Affidavit of Custody
This form is needed if a change in guardianship occurs for a current student.
2. Affidavit of Support
This form is needed if a change in guardianship occurs for a current student.
3. Affidavit of Residency
This form is needed if a change in guardianship occurs for a current student.
4. Enrollment Notification
This is LVPA’s initial application and is also required for change of address, or any personal information such as name change or school district change that affects the student.
5. Petition for Permanent Early Dismissal
For upper classmen to request an early dismissal privilege.
6. Record Release
Used to obtain records from a previous school or entity.
7. Community Service Log
Time sheet of student community service hours needed for LVPA’s graduation requirement.
8. Transcript Release
Form needed to send a student’s transcript to a college, University or scholarship entity.
9. Withdrawal
This form require to withdraw a student from LVPA
